The committee on finance is required to make provision for an annual audit of the records of all the financial officers (including the financial secretary or church business manager and treasurers) of the church and all its organizations and shall report to their district. Mississippi Conference plan members can print and take this form with you to your HealthyYou or Medicare annual physical appointment and have your physician’s office sign this form. You will qualify for a $75 cash incentive reward for completing a HealthyYou screening or a Medicare Annual physical. Up to three family members are eligible.
